Student Information - Please fill out the following form if you are registering a student new to Adams K-6thh grade.  

Birth Certificate (Original) - Office will copy and return the original.

Health Form -  Obtain at the school office to notify the school nurse of possible health concerns.

Immunization Record -  Bring your record to be transcribed to the official school form. Utah School Immunization Record

Proof of Residence -  Please refer to link for documentation needed. Residency Procedures!

Guardianship Status -  Please refer to link for documentation needed. Guardianship Status Document!

Homeless Assistance Act Questionnaire - Services your child may be receiving or has received.

 504 Plan -  If your student is working under a current 504 plan, please provide a copy.

Individualized Education Plan (IEP)- If your student has a current IEP please notify the office staff and provide a copy of the IEP if possible.

Request for Records -  Please provide the complete address, phone number and fax number to your student's previous school.

Please bring in the following documents to the office anytime in August or prior to your student(s) start date.
