Immunization Record - Bring your record to be transcribed to the official school form Utah School Immunization Record.

Immunization Requirements for Kindergarten Entry (for your information only, not a form).

Proof of Residency Form - Please refer to link for documentation needed.

Guardianship Status Form - Please refer to link for documentation needed.

Homeless Assistance Act Form - Required by the state to identify and offer assistance to qualifying students and their families.

Health Forms - For students with health concerns. Available at the school office.

504 Plan - If your student is working under a current 504 plan, please provide a copy.

Individualized Education Plan (IEP)- If your student has a current IEP, please notify the office staff and provide a copy of the IEP if possible.

Request for Records - Please provide the complete address, phone number and fax number to your student's previous school.

Please bring in the following documents to the office prior to your student's start date. We will make copies and return the originals to you.

New Student Registration:

Complete the following steps below to register your student at Burton Elementary:

1.  Make sure your student lives in Island View Elementary boundaries.

Students must live in the Island View Elementary School boundaries to register.

2.  Make sure your child's immunizations are up to date and you have proper documentation.

State law requires that each student's immunization records be up to date for the student to enter school. Immunization records must be turned into the school before your child will be allowed to start school. The immunization requirements are:

DPT 4 Shots (students need a 5th dose if they received the 4th dose before age 4)

ORAL POLIO 3 Doses

MMR 2 Shots

Hepatitis B 3 Doses (students born after July 1, 1993)

Hepatitis A 2 Doses (students born after July 1, 1996)

Chicken Pox (Students born after July 1, 1996)

3.  Complete and bring the required forms to the office. If you do not have access to a printer, you can pick up a registration packet from the office.
